Insight Rehab Consultants, LLC, is an independently owned and operated Case Management and Life Care Planning company established in 2005. As Registered Nurses, our primary concern continues to be that of patient care. Our goal is to provide our clients with quality, detail oriented, customized Case Management and Life Care Planning services that meet their individual case concerns by incorporating an interactive relationship with the involved parties.

If you prefer phone, email or fax communications; monthly or event driven reporting; limited or full services; we will work to fulfill our customer needs. We are proactive in maintaining the needed contacts to remain well informed of case activity and work diligently in the provision of quality services. Our geographic locations, with Donna Ziegler RN, CCM, CNLCP in the Fort Wayne area and Darlene Lowe RN, CCM in LaPorte County, provides easy access to the entire Northern Indiana territory.
